Centreview Church is planning an activity that requires your permission prior to participation. We have provided you with the details of the activity and request that you complete and sign the permission form. The safety of your child is our primary concern. DESCRIPTION OF ACTIVITY
Activity: Centreview Summer Camp
Location: Corpus Christi Seperate School - 4155 Elora Drive, Mississauga, ON
Dates: July 8-12
Time: 9:00am-3:30pm
Details of Activity: There will be a Bible lesson, games, music, arts and crafts, but most importantly, this will be an opportunity for your children to grow in their faith and create long lasting friendships. We are looking forward to this fun-filled week!
Cost: $105 per child - Please don't let this cost be a hinderance to you. If this payment is not feasible, please speak to Katie.

Our goal is to help kids be able to participate and feel welcomed. With this, we ask that if there is anything we should be made aware of so that we can help your child in the best way possible.
While every precaution is taken for the safety and good health, some sports and activities carry with them the inherent risk of personal injury beyond the risks associated with many of the recreational activities at Centreview Church. I understand and accept these risks and agree that by allowing my child to participate in those activities, he/she may be taking part in a recreational activity that presents the potential for personal injury.
I, the parent/guardian named below, authorize the Lead Pastor or one of Centreview Church's Ministry Personnel to sign a consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment or procedures for the participant named above.
I, the parent/guardian named below, undertake and agree to indemnify and hold blameless Centreview Church, its Ministry Personnel, its Pastors and Board from and against any loss, damage, or injury suffered by the participant as a result of being part of the activites of Centreview Church, as well as of any medical treatment authorized by the supervising individuals representing Centreview Church. This consent and authorization is effective only when participating in or traveling to and from events of Centreview Church.
I hereby grant permission for Centreview Church to record sounds, images, or video of my child while attending Summer Camp. I also give permission for Centreview Church at its sole discretion, to use these sounds, images, or videos in publications (including print, websites, and social media platforms) owned by Centreview Church in relation to their Summer Camp.
